Applicants are requested to download and fill out the following forms. The immunization and health record forms must be filled out by your child's pediatrician prior to your child starting school. Children may not begin school until these forms are returned, and forms are required to be updated every 12 months. By New Jersey law, failure to provide current records can result in child being excluded from school. Additional downloads include pertinent information on the enrollment of your child.
